Birth Certificate Request Form
To amend or correct a birth, death or marriage certificate, you have to contact the Clerk's office at 508-799-1121 or CC_Amendments@worcesterma.gov. A representative will give you information on the necessary evidence or paperwork you need to request a record correction. You must schedule an appointment with the Clerk's office to amend any vital records.
To correct a mistake on a birth, death or marriage record you will need to show evidence that the information was incorrect at or near the time of the event. In most cases, vital records list only what was true at the time of the birth, marriage or death, and updates are not made for changes that happened later. For example, you cannot change your birth record solely for a legal change of name.
You may search the Birth Record database for any birth that occurred within the city limits of Worcester, MA between January 1, 1900 and February 1, 2011 by entering the last name and first name of the individual.
If the birth record is not available online please contact the Office of the City Clerk for assistance.
Can I Request a Copy of the Original?
Yes, but additional fees will apply.
City Clerk staff give you an overview of the Vital Records process.
